Mozaic ElderCare Navigation is the most comprehensive program of its kind, providing personalized assessments, recommendations, and assistance.

Mozaic ElderCare Navigation helps you make the best decisions about all issues regarding your senior loved one, with the convenience of a single trusted go-to advisor that stays with you over time giving you the confidence and peace of mind in your decisions. A Mozaic Care Navigator provides advice on day-to-day issues, during transitions related to health or aging, and during emergencies. Care Navigators are local and can be on-site when needed.

Mozaic ElderCare Navigation offers subscription plans that fit your needs and budget. Monthly subscription provides ongoing care navigation and allows for personalized services as Care Navigators get to know you and your family. Mozaic ElderCare Navigation is also available on an hourly basis to support you during transitions and other critical times.

Mozaic ElderCare Navigation Can:

Provide coordination of resources, especially during care and health transitions
Make home visits to assess home safety and health needs
Evaluate in-home care needs
Arrange in-home care and select care personnel
Assist with other living arrangements
Provide consultation and advocacy in dealing with providers, benefits and services
Coordinate medical services and appointment
Address emotional concerns and provide supportive counseling
Provide long-term planning services such as assistance with
long-term care directives, living will, health proxy and powers of attorney
Communicates and coordinates with family members

ElderCare Navigators are available to discuss needs by appointment in-person or over the phone. Consultations will be scheduled at your convenience. Service agreements and terms will be reviewed.

Payment packages include single session planning meetings, hourly services, and monthly on-going assistance plans. We can assist in determining the most appropriate and cost-effective option for you.
